Empowered Aging: Advanced Care Planning

Advance care planning ensures your goals, values, and preferences are known, not only at the end of life, but also if you become unable to communicate those desires due to illness or injury. Participants will learn about end-of-life paperwork such as advance directives, durable Power of Attorney for Health Care, and portable orders for life-sustaining treatment (POLST).
