Join a multidisciplinary team from Whidbey Health to learn more about your options for Advance Care Planning. Advance Care Planning (ACP) is a way for you to express your goals of care anywhere along the health care continuum.

 

There will be a short PowerPoint presentation, a Q & A session, and an opportunity to talk one-on-one with a social worker, provider, or registered nurse (RN).

 

Please note that this is an informational session only; no medical care will be provided.

 

This event is part of the End of Life program series.
